If we are participating providers for your insurance company, you will be responsible for your co-pay or deductible at the time of service and then we will gladly file your claim for you. If we are not participating providers for your insurance company, you will be obligated to pay in full at the time of service and filing your own claim.

It must be understood that the patient, not the insurance company, is ultimately accountable for the payment of the bill. Those patients paying privately are required to pay for services when they are rendered. We accept VISA, MasterCard, American Express and Discover.

It is very important that you inform us immediately of any changes in your health insurance coverage. Please bring your insurance card with you to each office visit to ensure the office keeps your file and your coverage updated.
